Because we are at full capacity, Siloam Family Health Center is not accepting new patients until further notice. We apologize for the inconvenience.

BECOME A PATIENT
Siloam is a Christian ministry with the mission to share the love
of Christ by serving those in need through health care. All people
are welcome to come to Siloam, no matter what their faith tradition
or religious beliefs may be.
If you do not have any health insurance, you are eligible
to be a patient at Siloam. However, because of the volume of people needing affordable health care, new patients might experience a delay in getting an appointment. Click here for a list of other faith-based health centers in the Middle Tennessee area.
HOW TO GET AN APPOINTMENT AS A NEW
PATIENT:
Patients need to make an appointment to see the doctor. We do
not see patients who walk in without an appointment.
Call the clinic at (615) 298-5406, ext. 102 and a receptionist
will help you make an appointment. Please tell the receptionist
if you need an interpreter for your appointment. We will try to
have an interpreter here when you see the doctor. Some of our interpreters
are on staff, some are volunteers, and some are over the telephone.
You do not have to pay for this service.
PAYING FOR YOUR CARE:
Siloam is not a free clinic. Patients are asked to pay $40 to
see the doctor.
This $40 does not include the cost for laboratory tests or medical
procedures that are sometimes necessary.
Patients who cannot pay $40 can apply to Siloam's sliding fee
program. The sliding fee program allows you to pay a reduced fee
based on family size and income.
If you apply to the sliding fee program, you will need to bring
proof of income on your first visit.
No one is turned away if they cannot pay.
